Your health is important to you — all of the time. That’s why Froedtert & the Medical College of Wisconsin offer MyChart®. With this Internet connection to your doctor’s office, you can view appointments, see your health history, get test results and renew prescriptions online — any time!
MyChart® is a free, easy and secure way to view your health information and communicate with your healthcare team at the primary care clinics of Froedtert & the Medical College of Wisconsin. All you need is Internet access and e-mail.

MyChart® users can be assured that communications between you and your clinic healthcare team are carried over a secure, encrypted connection directly into the electronic medical record.
How to EnrollYou enroll in MyChart® at your clinic. Simply ask for an enrollment form at the clinic reception desk. This form is also available online at www.mychartlink.com to print, complete and bring to the clinic. Once you enroll, you’ll receive a code to activate MyChart®. You then create your own username and password.
Urgent Medical MattersMyChart® is not intended for sending messages requiring urgent attention. For urgent medical matters, contact your doctor’s office by phone.
