Convenient and secure access to your electronic health record

Your health is important to you — all of the time. That’s why Froedtert & the Medical College of Wisconsin offer MyChart®. With this Internet connection to your doctor’s office, you can view appointments, see your health history, get test results and renew prescriptions online — any time!

MyChart® is a free, easy and secure way to view your health information and communicate with your healthcare team at Froedtert & The Medical College of Wisconsin. All you need is Internet access and e-mail.

You access MyChart® with a user code and password that you create, ensuring your health information is secure.

You can also take MyChart® with you wherever you go with a new MyChart iPhone app!

Connecting to MyChart®
Once you have access to MyChart®, it’s easy to view your information. Simply:

Log in at www.mychartlink.com or
Visit www.froedtert.com or www.mcw.edu and click the MyChart® link on the homepage.

With MyChart®, you can:

Review your current health issues, medications, immunizations and allergies
View test results
Renew medication prescriptions
View your medical history
Communicate with your healthcare team
Request, cancel and view appointments
Access health and appointment information for family members

Our policies and procedures regarding access to and release of medical records conform to state and federal laws and are designed to safeguard your privacy.

MyChart® users can be assured that communications between you and your clinic healthcare team are carried over a secure, encrypted connection directly into the electronic medical record.

How to Enroll

You enroll in MyChart® at your clinic. Simply ask for an enrollment form at the clinic reception desk. This form is also available online at www.mychartlink.com to print, complete and bring to the clinic. Once you enroll, you’ll receive a code to activate MyChart®. You then create your own username and password. Additional paperwork is required for access to family member health records and appointments.

Urgent Medical Matters

MyChart® is not intended for sending messages requiring urgent attention. For urgent medical matters, contact your doctor’s office by phone.

My Froedtert

MyChart® should not be confused with My Froedtert. My Froedtert is a tool we offer to our visitors to manage their e-mail subscriptions to My Health News.
