MyChart® — Convenient and Secure Access to Your Health Information


You’ve just visited a Froedtert & The Medical College of Wisconsin primary care clinic. Now you want to know your test results or check a future appointment time.

Simply turn on your computer and connect to MyChart® for free, easy and secure access to portions of your electronic medical record. If you have Internet access and e-mail, you can view your health and appointment information, view information for family members, manage appointments, send messages to your healthcare team and perform other functions— when it’s convenient for you!

You may enroll in MyChart® at your Froedtert & The Medical College primary care clinic. Simply ask for an enrollment form at the reception area. This form is also available at mychartlink.com to print, complete and bring to the clinic.
