Handbook for Instructors/Students - This document must be reviewed by all instructors/coordinators and students each semester. It is the responsibility of the instructor/coordinator to make sure that the students receive the handbook content to review.
Affiliation Agreement - This sample Affiliation Agreement is available for reviewing student/instructor contract information.
Program Memorandum - This sample Program Memorandum is available for reviewing student/instructor contract information.
Instructor/Coordinator Agreement - The Instructor/Coordinator Agreement must be completed and returned by each instructor/coordinator who is responsible for students annually prior to the start of the first clinical.
Safety Quiz - The Safety Quiz must be completed and returned by each instructor/coordinator who is responsible for students annually prior to the start of the first clinical.
Confidentiality & Security Agreement - Confidentiality & Security Agreement must be completed for each student (fax to Jane Hendricks at 414-805-6920).
Supervision and Cosignature Requirements - For Nonphysician Students Interns and Externs.
Parking Information
Student Identification - all students must wear school ID badge at all times when on Froedtert campus unless provided with a Froedtert ID badge, then Froedtert badge must be worn.

Information for Observers

An observer is an individual who is 18 years of age or older who is allowed to observe patient care activities at Froedtert Hospital and Clinics. An observer must meet the established requirements identified in the Observer Policy. See the policy and attachments for specific information. The requestor must have an identified staff member that has agreed to the observe/shadow experience. For specific information on Observers in the Operating Room and Trauma Room, see the attachments.
