PGY1 Residency Project
Project PRIMED (Pharmacist Reconciliation and Medication Education at Discharge)
Goals of the project: 1) improve patient safety by demonstrating that pharmacists can reduce medication errors upon hospital discharge, 2) improve patient access to outpatient prescriptions by eliminating common barriers that delay the filling of discharge prescriptions, and 3) financially justify additional pharmacists for hospital-wide implementation of Project PRIMED by generating revenue for the onsite outpatient pharmacies.

Reducing readmissions after a change in pharmacy practice model

Why did you choose to pursue a residency?
I chose to pursue a residency in hospital to further advance my clinical skills, apply the knowledge I’ve learned at the University of Minnesota, and establish the foundation for a successful career. I will continue to gain experience in a variety of clinical settings, which will help me identify specific areas of interest, and will allow flexibility to adjust a developing career path.

What attracted you to Froedtert’s pharmacy residency program?
I wanted to complete my residency training at a large academic medical center that focuses on continuing education, and continually strives to improve their pharmacy services. Froedtert’s pharmacy staff are friendly, energetic, and motivated. I also liked the multitude of flexible clinical rotations available, that allow participating in care-coordinated rounds, with focus on critical care.

Professional interests
I would like to become a clinical pharmacy manager or clinical coordinator within a large health-system, which will allow for further learning and mentorship. I’m most passionate about ambulatory clinical management. Currently, ambulatory pharmacy is a rapidly evolving field of practice for a variety of reasons (expanding MTM services/reimbursement, establishing collaborative practices, attempting to obtain provider status, specialty pharmacy, mail order pharmacy, PBMs, MCOs, ACO’s, etc.). With all this change, managers/leaders will be essential to shape the future of the pharmacy profession.

Hobbies
Tennis, running, traveling the world, and most recently mountain biking.

What's your favorite thing to do in Milwaukee?
Summerfest and Brewers games.
