New federal regulations require that additional steps be taken to maintain patient confidentiality. Following these regulations, Froedtert Hospital is required to identify entities that receive patient identifiable information to provide products or services to the hospital. Those who meet the criteria are considered business associates and are required to sign agreements before receiving the confidential information. View a Business Associate Agreement (PDF).
If it has been determined that your company is required to have a Business Associate Agreement on file with Froedtert Hospital, please forward two (2) signed copies of the Business Associate Agreement to:

When the BAA is returned to Froedtert, please clearly indicate in a cover letter who should receive the signed copy back, and the address it should be sent to. Once fully executed, a signed copy will be sent back to the company.
Once it has been determined that your company is required to have a Business Associate Agreement, we will be unable to process checks to your company until the Business Associate Agreement is in place.
